Mental health case management

For people living with a serious and persistent mental illness, an assigned case manager coordinates referrals and services to address a person’s mental health needs, including medical, social, educational, and vocational services. Services are individualized and focused on supporting healthy living. Individuals must meet the criteria for case management, since there is limited availability of services that also vary across states. Mental health case management is offered through a variety of community organizations that specialize in mental health services.
